Responsibilities: As a Nurse Practitioner with CommonSpirit Health at Home, you'll do more than assess symptoms - you'll bring comfort, clarity, and compassionate care to patients during some of the most meaningful times of their lives. This role is your chance to truly connect with patients and families, offering clinical expertise while honoring their dignity and individual needs. In collaboration with our Medical Director and interdisciplinary care team, you'll play a key role in evaluating ongoing eligibility for home health and hospice care. You'll deliver personalized assessments, build trusting relationships, and ensure our patients receive the right care, in the right setting, at the right time.
