Responsibilities:

Are you a highly skilled RN passionate about holistic patient care and seamless transitions? Do you thrive in a collaborative environment where your critical thinking optimizes outcomes and enhances patient experience?

Mercy Medical Center seeks a dedicated RN Care Coordinator to drive care progression and discharge planning. You'll be the advocate, navigating complex systems and collaborating with multidisciplinary teams to empower patients on their journey to well-being.

You will:

* Assess & Plan: Complete and document discharge planning assessments, reassessing as needed.
* Facilitate & Oversee: Develop and evaluate multidisciplinary discharge plans, engaging patients, families, and post-acute providers.
* Coordinate Care: Collaborate with the team to ensure care progression and appropriate utilization of inpatient resources using evidence-based guidelines.
* Transition Expert: Ensure timely and smooth transitions to appropriate post-acute services based on patient needs/choices.
* Mitigate Risk: Identify readmission risks and implement interventions for at least 30 days.
* Educate & Advocate: Deliver patient notifications, provide education, and advocate for patient choice and needs (cultural, language, socioeconomic).
* Improve & Adhere: Participate in performance improvement; adhere to all policies, standards, and regulations.

Shift: 8am - 4:30pm

Core Schedule: Week 1 Mon, Tue, Fri, Sat; Week 2 Sun, Wed, Thu, Fri

Qualifications:

What You'll Bring:

* Knowledge of Regulatory Standards: Understanding of CMS standards and requirements.
* Required Education & Experience:
* Graduate of an accredited school of nursing.
* Minimum two (2) years of acute hospital clinical experience OR a Master's degree in Case Management or Nursing field in lieu of 1 year experience.

* Required Licensure:
* Current RN license in Oregon.
* AHA BLS for Health Care Providers within 3 months of hire.

* Preferred Qualifications: BSN, 5+ years nursing experience, CCM/ACM-RN/UM Certification, knowledge of managed care/payer environment.
