				Overview:

Bilingual required with the ability to pass bilingual testing from a Confluence Health Vendor within 90 days of start date. 

he Hospitalist Nurse Navigator is responsible for care coordination activities related to outpatient care, patient follow-up, patient education, and patient experience with particular focus on Spanish-speaking patients. Works collaboratively with the Hospitalist Medical Director and the Hospitalist APP Supervisor to improve patient outcomes.   In addition, they are responsible for development of standard work for ongoing measurement of metrics and quality indices.  Due to the clinical complexity of the inpatient environment, the Hospitalist Nurse Navigator will assist in optimizing performance on defined key metrics by coordinating and overseeing clinical performance activities, including patient specific initiatives and enhanced communication through the care team related to patient flow and experience of care.  This position will conduct any needed chart audits and work in conjunction with Care Management, Quality Reporting and Data Management and Analytics.

Position Reports To: CWH Administrator

Responsibilities:

* Attends daily multi-disciplinary rounds and establishes collaborative rapport with multidisciplinary team.
* Ensures timely outpatient follow-up for all patients discharged from service. This may include but not be limited to Primary and Specialty providers, Home Health & Hospice, DME suppliers, and outpatient Case Management.
* In conjunction with Care Management, facilitates and/or conducts patient, family, and team conferences to develop and monitor a plan of action as necessary. This includes patient education on the disease, treatment options, procedures and medication regiments and side effects.
* Works directly with Spanish-speaking patients and families to ensure shared understanding of information and goals.
* Conducts accurate medical record review, abstraction of specific data, and ability to enter data into computerized abstraction tools for organization-wide quality improvement activities and regulatory reporting requirements.
* Assists Hospitalist Medical Director & APP Supervisor in development of departmental scorecard and responsible for data population.
* Attends and participates in all departmental meetings.
* Utilizes Microsoft Word, Excel, and PowerPoint to effectively communicate performance and assist with identification of improvement opportunities.
* Assists with coordination and insuring timely completion of death certificates.
* Deals appropriately with sensitive confidential information contained in events, medical records, minutes, letters reports and other documents.
* Performs other duties as assigned.

Qualifications:

Required:

* At least two years recent successful experience in an acute-care setting.
* Must possess basic computer skills related to Microsoft Office products.
* Effective teaching, critical thinking, and clinical skills.
* The ability to function autonomously in a non-structured environment.
* Current licensure in the state of Washington (RCW 18.88) or licensure through Multistate Nurse Licensure Compact (SSB 5499) with Bachelor of Science in Nursing. May consider qualified applicant able to achieve BSN within 3 years of hire.

Desired:

* Master's Degree in nursing or related field.
