Overview:
St. Luke's Health system is looking for a Nurse Practitioner or Physician Assistant to develop our Hospital at Home program. By extending acute-level care to patients in their homes, Hospital-at-Home programs have demonstrated significant cost savings, improved patient outcomes, and elevated patient satisfaction. This is a groundbreaking program where medical-surgical care is provided in the home. This is the first of its kind in Idaho. Being inspired to help build this program and adapt as it changes is critical.

Team Collaboration:

Primary Functions:
* Source potential admissions from the emergency department or inpatient units
* Complete admissions with a virtual attending physician
* Support inpatient care through onsite, virtual, and in-person visits
Responsibilities:
* Developing Role: source and admit patients for a home-based inpatient care
* Evolving Role: opportunities for daily rounding and in-person home visits as the program grows
What to Expect:
* Growth Mindset: Participate in a developing the program.
* Flexible Hours: 7am-7pm with some swing shifts up to 10pm, no night call currently, possible in the future. Some weekends, evenings, and holidays required. APPs participate in a self-scheduling process that allows for flexibility.
* Locations: Team members are expected to rotate through our three Treasure Valley hospital sites (Boise, Meridian, Nampa) and are typically assigned one site during their shift with potential to cover more than one site as needed.

Key Skills:
* Patient evaluation, acute care assessment, defining an assessment and plan, and completing documentation in electronic medical record
* Prefer experience with caring for acutely ill patients and admitting them to hospitals

Responsibilities:
The Nurse Practitioner (NP) is responsible for promotion of health, prevention of illness, and the response of the human condition through diagnosis and treatment. The NP works under the supervision/sponsorship of designated physician(s) and in collaboration with a team of other providers and staff; promoting a culture of collaboration and support between all providers within the St. Luke's Clinic Medical Group and to support the model of team based, patient-centered care endorsed by physicians and advanced practicing providers within our medical group.
-Provide care through diagnosis, clinical judgment, anticipatory guidance, and management to restore, maintain, and improve the health status of the patient. -Perform history and physical exams, acute and episodic care, orders pertinent lab and diagnostic studies with interpretation and consultation. -Initiate and manage appropriate pharmacologic, non-pharmacologic agents, and treatments within the scope of practice. -Establish and document the healthcare plan and treatment, maintaining records of each case that are sufficiently complete for any healthcare professional to evaluate and follow the treatment plan and subsequent outcome(s). -Recommends strategies to assist SLHS in meeting its critical goals and objectives in the most efficient manner possible; administers recurring or daily functions or processes necessary for systems execution and identifies opportunities for improvement or efficiencies.'-Interact with patients/families in a professional and sensitive manner, and maintain confidentiality. -Maintain professional relationships and convey relevant information to other members of the health care team within facility and any applicable referral agencies. -Accept assignments based on patient census, acuity, needs, and the qualifications and competencies of self and of other staff members. -Attend staff meetings, in-services, and continuing education. Report to work on time and as scheduled. -Contribute to annual reviews of peer performance as requested, assist in data collection, study methods and other development as assigned. -Perform other duties and responsibilities as assigned, or as pertains to the position and specific department/specialty.
Qualifications:
* Education: Graduate education from an accredited Nurse Practitioner or Physician Assistant program or qualifying education as designated by the Idaho Board of Nursing or Medicine.
* Experience: previous Home Health or Hospitalist Advanced Practice Provider experience strongly preferred
* Licenses/Certifications: License and certification, as required by the specialty, or be eligible to obtain licensure from Idaho Board of Nursing or Medicine to practice as a Nurse Practitioner or Physician Assistant. Must obtain and maintain unrestricted prescriptive authority, DEA, and controlled substance licenses.
