Overview: From primary to specialty care, as well as walk-in and virtual services, CHI Health Clinic delivers more options and better access so you can spend time on what matters: being healthy. We offer more than 20 specialties and 100 convenient locations; with some clinics offering extended hours. Responsibilities: $2,500 annual CEU benefit plus uncapped productivity bonuses * Evaluates and assesses the bio-psycho-social needs of patients. * Assists in determining the most appropriate level of care for the patient, and makes appropriate referrals when indicated. * Develops comprehensive treatment plans in accordance with policy and procedures. * Provides supervision to clinical staff as needed. Qualifications: * A minimum of a Masters degree from an accredited educational institution in Counseling, Social Work, Nursing or other related behavioral health field is required. * LIMHP or LISCW license required
