Overview: Responsible for completing psychosocial assessments and coordinating the interdisciplinary patient care plan for mental and behavioral health patients. Acts as a liaison between a patient or family and health care personnel to ensure necessary care is provided promptly and effectively. Works with internal and external resources to coordinate a timely and safe transition of patients to the appropriate level of care. Responsibilities: * Conduct psychosocial and diagnostic assessments, and diagnose according to DSM guidelines. * Deliver psychotherapy services (individual, couples, family, group) and crisis interventions, including for substance use. * Use evidence-based practices and follow ethical, regulatory, and culturally appropriate standards. * Include family or collateral sources in treatment and maintain patient-informed communication. * Create, update, and document treatment and safety plans; ensure accurate and timely records. * Make referrals and coordinate care (e.g., pharmacy, outpatient, SDOH needs, transportation). * Educate patients on discharge planning and post-acute care services. * Ensure timely follow-up post-discharge (e.g., within 7 days). * Participate in training, meetings, and maintain communication with teams and patients. * Support peer-led groups and help meet Certified Community Behavioral Health Clinic (CCBHC) standards. Qualifications: Education: Master's degree in Social Work or Counseling Master of Science Degree in Social Work required Professional Counseling or Marriage and Family Therapy with a clinical/mental health concentration from an accredited school preferred Experience: 2 years relevant social work and/or counseling experience in a health care setting required License(s)/Certification(s): LMSW, LISW, LMHC, or LMFT required May be required to obtain BLS within six months of hire for certain sites. Knowledge/Skills/Abilities: Excellent communication skills, knowledge and experience in good interviewing strategies. Demonstrate knowledge, skill and ability to provide services to child, adolescent, adult and/or geriatric population. Individual will demonstrate effective written and verbal skills allowing communication with physicians, co-workers, other clinic/hospital personnel, as well as the public. Position requires good organizational skills.
