Responsibilities:

* Perform diagnostic assessments and gather essential clinical information to create individualized, evidence-based treatment plans tailored to each client's unique needs.
* Offer one-on-one and group counseling sessions focused on problem-solving, emotional support, and connecting clients with vital community resources.
* Work alongside physicians, nurses, social workers, and other care team members to develop and refine care strategies that promote recovery and well-being.
* Continuously evaluate client progress with follow-up assessments, refining treatment goals to reflect clinical findings and changing needs.
* Be a calm and confident presence during moments of crisis, providing immediate intervention, clear guidance, and stabilization strategies.

Qualifications:

Education:

* Requires a high school diploma or GED and a minimum of five years documented clinical experience in mental health or human services; or
* An individual possessing a certificate of psychiatric rehabilitation from a DHS-approved program, plus a high school diploma or GED, plus two years' documented experience in providing mental health services or
* An individual possessing a bachelor's degree in counseling and guidance, rehabilitation counseling, social work, education, vocational counseling, Psychology, pastoral counseling, family therapy, or a related human service field.

License(s)/Certification(s):

* Valid driver's license.
* (CPI) training within 6 months of hire.
* Obtain certification within 2 months of hire and re-certify annually according to the requirements set forth in the Illinois Department of Healthcare and Family Services related to the Illinois Medicaid Comprehensive Needs & Strengths (IM+CANS)
