Position Summary:
We are seeking a dedicated and detail-oriented Anticoagulation Pharmacist to join our pharmacy team in Boise, Idaho. This full-time position plays a vital role in the safe and effective management of anticoagulant therapies, ensuring optimal patient outcomes through clinical expertise, collaborative care, and patient-centered education.

Key Responsibilities:

* Review the appropriateness and safety of prescribed anticoagulant medication therapy.
* Provide accurate and timely medication information and consultation to patients, nurses, physicians, and other medical staff.
* Interpret prescriptions, compound and dispense medications in accordance with regulatory standards and institutional protocols.
* Supervise and authorize the activities of pharmacy technicians, externs, interns, and other support staff.
* Maintain complete and accurate records related to medication therapy, prescriptions, patient billing, medical documentation, and inventory control.
* Deliver cognitive clinical pharmacy services, including assessment and documentation of interventions, aligned with medical center standards.
* Administer medications and vaccinations when appropriate, in compliance with applicable laws and institutional policies.
* Assist in training and development of technicians, externs, interns, residents, and other department staff.
* Serve as a mentor and role model for pharmacy students, residents, and early-career practitioners.

Qualifications:

* Doctor of Pharmacy (PharmD) degree from an accredited institution.
* Current pharmacist licensure in the state of Idaho, or eligibility to obtain prior to employment.
* Completion of a PGY1 Pharmacy Residency (PGY2 preferred, especially in Ambulatory Care or Anticoagulation Management).
* Strong knowledge of anticoagulation pharmacotherapy, monitoring parameters, and clinical guidelines.
* Excellent communication, collaboration, and patient education skills.
* Ability to function effectively within an interdisciplinary healthcare team.

Preferred Experience:

* Prior experience in anticoagulation clinic or ambulatory care setting.
* Board Certification (e.g., BCACP, BCPS) is a plus.
* Experience with electronic health records (EHR) and pharmacy information systems.

Work Environment:
This position is primarily based in a clinical setting and involves direct patient interaction, medication preparation, and collaboration with multidisciplinary teams.

Responsibilities:

The Pharmacist is responsible for the delivery of pharmaceutical care to patients and customers of the medical center.

* Review appropriateness of prescribed medication therapy. 

* Provide medication information and consultation to patients, nurses and medical staff. 

* Interpret prescriptions, compound and dispense medications. 

* Supervise and authorize the work of technicians, externs, interns and support staff. 

* Prepare and maintains all necessary records on medication therapy, prescriptions, patient charges, medical records and inventory. 

* Perform cognitive clinical pharmacy services and document findings and actions in accordance with standards of the medical center. 

* Administer medications as appropriate.

* Administer vaccinations as appropriate.

* May assist in the training of technicians, externs, interns, residents and other department staff. 

* Serve as a role model and mentor to technicians, students, residents and new practitioners. 

* Perform other duties and responsibilities as assigned.

Qualifications:

* Education: Degree from an accredited pharmacy program
* Experience: 0 years' experience 
* Licenses/Certifications: Valid Pharmacist License in state of practice AND Current Basic Life Support (BLS) Provider Certified through American Safety and Health Institute, American Heart Association, or American Red Cross.
