Overview: Unity Point St. Luke's - Downtown Full Time, Days/Evenings Hours between 7a-8:30p, shifts may include 7a-5:30p or 10a-8:30p $3,000 Sign-on bonus Under supervision of the Pharmacist, the Medication Reconciliation Specialist is responsible for obtaining a current and accurate home medication record on newly admitted inpatients through information obtained from the patient, family, home pharmacy, physician office, or other facility as required. Inputting correct home medication information, dose, frequency, time of last dose, home pharmacy, and detailed allergy information into Epic. Historians will complete at least 14 histories per shift, and will also complete reconciliation for patients requested by the Hospitalist Service. Responsibilities: * Obtain names of newly admitted patients via ER Track Board and/or Nursing Bed Manager, and upon request by Nursing/Hospitalists. * Interview newly admitted patient or patient's family to obtain the most accurate and current list of home medications the patient is taking. It is critical that the Historians create a positive and safe environment to gather best med information. * Contact patient's family, nursing facility (MAG or MAR obtainment), home pharmacy, and/or physician office for medication clarifications as necessary. * Input best and most accurate obtained home medication list into the EMR. * Obtain and input into EMR information on what primary home pharmacy they utilize and any secondary pharmacies they may use. * Obtain and input into EMR a detailed description of drug allergies and what reaction occurred due to that allergy (i.e. allergy = morphine which causes reaction = shortness of breath). * Obtain current Pneumococcal and Flu vaccines and communicate with staff if deficient with immunizations. Also any herbal and over the counter medications routinely taken. * Display competent computer skills in into EMR and great interviewing abilities with and probing tactics to get the most accurate medication information from the patient and family. * Be willing to assist with audits and analyze data to promote quality care in the nursing departments aligning with medication reconciliation, pain management documentation, and all medications signed off in Omni and the EMR. Qualifications: Education: * High School Diploma or Equivalent required. Experience: * Two (2) years of related work experience or education to replace work experience preferred. License(s)/Certification(s): * Certified Pharmacy Technician Certification required. * Registration with the State of Iowa Board of Pharmacy required.
