Overview: Centurion is proud to be the provider of comprehensive healthcare services to theTennessee Department of Correction. We are currently seeking a full-time Medication Assisted Treatment Care Coordinator to join our team at West Tennessee State Penitentiary located in Henning, Tennessee. Responsibilities: The Medication Assisted Treatment Care Coordinator works as team member to coordinate team-based, inmate/patient centered, care continuation services for releasing inmates in the Vermont Correctional Medication Assisted Program. Coordinated care with HUB Clinic's partnerships is required. The MAT Care Coordinator consults with the Addiction Specialist Physician. The role reports to the Director of Care Coordination. Qualifications: * Preferred Nursing Licensure -Licensed Nursing Assistant or a Licensed Nursing Professional in the state of Tennessee * Experience with the correctional health care * Experience with detailed data reporting * Proficient in verbal and writing communication * Ability to determine appropriate actions in complex care follow-up needs * Strong Organizational Skills and ability to prioritize duties
