Overview: Presbyterian is seeking a Medical Social Worker to evaluate develop and implement appropriate discharge plans which consider the psychosocial and financial needs of patients and their families and functioning as a key member of the interdisciplinary healthcare team. This is directed at supporting and maximizing the social functioning and adjustment of the patient and their family Type of Opportunity: Part Time (0.6 FTE/24 hours per week) Work Schedule: Monday - Friday 8 hour days How you grow, learn and thrive matters here. * Educational and career development options, including tuition and certification reimbursement, scholarship opportunities * Shift differentials for nights and weekends * Differentials for higher education, certifications and various lead roles * Malpractice liability insurance * Loan forgiveness through the New Mexico Higher Education Department * EPIC electronic charting system Responsibilities: * The medical social worker is responsible for the identification and assessment of psychosocial problems and will work with patients and families achieve resolution or will provide referrals to the appropriate agencies for the service. * The medical social worker shall be available for patients and their families through many aspects of care. This includes psychosocial problem-solving activities in admission and discharge phases and assessment of financial needs and available resources, which contribute to a safe and appropriate discharge to the community setting. * Results of assessments and interventions are documented in the medical record. * The medical social worker functions as an advocate and liaison to the multidisciplinary healthcare team and is responsible for discharge plans, which will achieve optimal patient outcomes. * The medical social worker collaborates as a key member of the multidisciplinary healthcare team. Collaboration includes the active participation of the social worker in the team decision-making process. He/she is responsible for helping the team members understand function and role of the social worker in the clinical setting. * The medical social worker acts as an expert resource and contact for the multidisciplinary team, including other social workers or community case managers, physicians, ancillary staff and nursing staff on a multi-state and regional basis to help promote the optimal continuity of care for the patient in discharge or transfer arrangements. * Facilitates effective communication between administration, staff, physicians, other department, patients, families and the general public. Participates in conflict resolution as necessary and acts as an effective problem solver. * Exemplifies a high standard of leadership qualities and professionalism. Mentors the staff in developing and achieving professional goals. * Performs Utilization Review * Performs other functions as required. Qualifications: * Masters in Social Work required. * Must hold current License form the State of New Mexico as a Licensed Independent Social Worker or Licensed Masters Social Worker. * Experience or preceptorship in clinical social work is desirable
