				Overview:

The Manager of Nursing is responsible for the RN functions and interfaces directly with senior management to ensure that assessed member needs are effectively addressed. The Manager of Nursing serves as a valuable resource to the Care Management team by documenting critical clinical caseloads, reassessments, and change in condition information in the electronic record system. The Manager of Nursing partners with the RN team by coordinating services and community resources and meeting member socioeconomic needs to support the quality of life.

Responsibilities:

1. Manages a team of care management nurses and reviewers to ensure the seamless and timelycompletion and quality of care management while also focusing on quality, gaps in care, andutilization.2. Assist the Director of Nursing in planning, developing, organizing, implementing, evaluating,and directing the day-to-day functions of the RN Care Management Team3. Makes written and oral reports/recommendations to the Director of Nursing asnecessary/required, concerning the operation of the RN Care Management Team4. Participate in the utilization review process and evaluate to determine if the member'scondition and needs meet criteria for covered services.5. Delivers member specific metrics and coaches RNs on gap closing opportunities.6. Collaborates with RN Care Management Team to improve performance in areas of Quality, RiskAdjustment and Operations7. Compare assessment data to baseline assessment to monitor Members' progress, recognizealterations in function, including life-threatening situations, and intervene and documentappropriately.8. Serves as clinical point person in response to Care Delivery questions, concerns, or issues9. Ensure that documentation in the care management record meets all applicable professionalstandards, using an EMR for each observation, verbal report, or interaction with the Member,Member's caregiver/family, PCP or other provider, whether by home visit, telephonic, or writteninteraction.10. Participate in Disease Management, Utilization Management (as required), and QualityImprovement activities.11. Utilize electronic timekeeping system as directed.12. Arrive to work on time, regularly, and works as scheduled.13. Recognize and follow the dress code of the facility, including wearing name tag at all times.14. Follow policy and procedure regarding all electronic devices, computers, tablets, etc.15. Support and abide by Elderwood IPA's Mission, Vision, and Values.16. Abide by Elderwood IPA's businesses code of conduct, compliance and HIPAA policies.17. Performs other duties as assigned by supervisor, management staff, or Administrator

Qualifications:

*  Current New York State Registered Nurse License and a valid NYS Driver'slicense in good standing required
*  Masters in relevant field preferred, or BSN with extensive experienceaccepted
*  Minimum of four (4) years nursing experience in home care, casemanagement, discharge planning or managed care required
*  Minimum of one (1) years management experience required
*  Minimum of one (1) year experience working with a frail or elderly populationrequired
*  Minimum of one (1) year experience with health assessments required
*  Bilingual skills are preferred
