				Overview:

Schedule: Full time Monday - Friday; leader on call rotation

Location: Provide leadership over team at Presbyterian Medical Center and Charlotte Orthopedic Hospital, office located in Charlotte Orthopedic Hospital; Presbyterian is our 661 bed level 2 trauma center centrally located just outside of uptown Charlotte; Charlotte Orthopedic Hospital is the region's only orthopedic hospital, centrally located just outside of uptown Charlotte

This role is ideal for an experienced case management leader looking to lead a high-performing, collaborative team.

Responsibilities:

* Oversight and management of case management team as a co-manager
* Operations, quality, productivity, and efficiency for case management processes
* Coordinate with facility and service line leaders to ensure continuity of care throughout the contiuum
* Connect with community resources and leaders to establish and maintain relationships

Qualifications:

* Education: High School Diploma or GED, required. 4 Year / Bachelors Degree Bachelor's Degree in Nursing or Bachelor's Degree in Social Work accredited by the Council on Social Work Education or Bachelor's Degree in related healthcare field, required. Graduate Degree, preferred. 
* Experience: Three years experience in Case Management or related experience, required. Leadership experience, preferred. 
* Licensure/Certification: If RN, current RN licensure in appropriate state, required. Certification in Case Mgmt. or related field, preferred. 
* Additional Skills (required): Current and future trends/practice in area of responsibility; regulatory and governing standards, policies and procedures; information systems and software used in area of responsibility; equipment used in performing assigned duties; application of safety and infection control policies and procedures; skills in interviewing and selection process; understanding of budgeting, staffing, payroll and purchasing processes; implementation of competency, feedback, development and progressive discipline processes; teambuilding; conflict management, and interpersonal relationship skills; principles of adult learning and teaching methods/models; decision making, project leadership and consultation; delegation and communication skills; understanding of customer needs and expectations and knowledge for improvement to exceed customer expectations. Develops and continuously improves discharge planning core processes. 24/7 department accountability.
* Additional Skills (preferred):Knowledge/Competence of Microsoft office products and keyboarding.
