Overview: Office Location: Woodbury, MNCoverage area: Southeastern Metro area Schedule: Monday - Friday, 8:00 AM - 5:00 PM Hospice of the Midwest is seeking a passionate, dedicated Social Worker to join our team in Saint Paul, MN. Our Social Workers provide emotional support, counseling, and resource coordination to patients facing terminal illness. This role plays a key part in the delivery of holistic care and ensures that patients' psychosocial needs are met with dignity and respect. Meets personnel qualifications stated in 418.114(b)(3). * Minimum of one (1) years' experience in health care, hospice experience preferred. Understands hospice philosophy, and issues of death/dying. * Experience in a hospice care preferred. * Demonstrates good verbal and written communication, and organization skills. * Current licensure according to Medicare guidelines to practice in state of operation. * Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
