				Overview:

CHI St. Alexius Health Garrison includes the services of a 22-bed critical access hospital, a 26-bed skilled nursing facility, an attached rural health clinic, and an Emergency Department that is available 24 hours a day. We offer many outpatient services, including physical therapy, radiology services, laboratory services, cardiac rehabilitation, and IV treatments. Since opening in 1952, CHI St. Alexius Health Garrison has been dedicated to serving the residents of Garrison and the surrounding rural communities. The hospital is equipped with 4 emergency rooms, on-site lab, radiology, CT and physical therapy services. Our patients have access to 24/7 emergency care, swing-bed program, and acute care services. 

CHI St. Alexius Health Turtle Lake is a 25 bed critical access hospital with a Level V Emergency Department that is available 24 hours a day. Our hospital has been committed to providing patients quality medical treatment in the Turtle Lake area and surrounding communities since 1952. Through the years, the hospital has sought to continually upgrade the quality and numbers of services it provides. The hospital's affiliation with major medical centers and health care agencies has allowed it to expand its health care services through a network that begins at the local level. CHI St. Alexius Health Turtle Lake is equipped with 2 emergency rooms, on-site lab, radiology, CT and physical therapy services. Our patients have access to 24/7 emergency care, swing-bed programs, and acute care services. 

Responsibilities:

This position will serve both Garrison and Turtle Lake facilities.

As our Social Worker you will proactively assess, develop, and implement comprehensive plans of care in collaboration with patients, families, and an interdisciplinary team. Every day you will complete psychosocial assessments, interview patients, relatives, friends or significant others to obtain information about the patient's personal, social and emotional history. To be successful in this role, you must provide therapeutic care to patients to assist them in accomplishing treatment goals using interventions and techniques that are appropriate to the age and developmental level of the patient.

* Developmentally Appropriate & Holistic Care: Deliver patient-centered care by assessing the unique needs of diverse populations-including those with physical, mental, or chronic illnesses-and facilitating safe, continuous transitions from admission through discharge
* Complex Social Needs & Crisis Intervention: Identify and manage social determinants of health (e.g., housing, food, transportation) and handle sensitive escalations, such as abuse, neglect, substance use, and psychiatric crises, through timely screening and referrals
* Advocacy, Education, & Patient Support: Provide ongoing support and education to patients, families, and the healthcare team regarding health management, community resources, and the navigation of social services
* Interdisciplinary Care Coordination: Lead and participate in patient/family care conferences to ensure seamless communication, identify appropriate decision-makers, and coordinate care plans across the clinical team
* Resource Management & Community Networking: Maintain up-to-date knowledge of community resources and insurance programs (Medicare/Medicaid/SSI), building partnerships to support life-care planning and effective transitions to post-acute or hospice care
* Regulatory Compliance & Documentation: Ensure thorough, timely documentation and adhere to all federal, state, and accreditation standards (e.g., Joint Commission) to optimize patient outcomes, reduce readmissions, and meet organizational financial goals

Qualifications:

Required 

* Bachelor's degree from a school of social work accredited by the Council of Social Work Education 
* Licensed Baccalaureate Social Worker (SWORKER:ND)

Preferred 

* At least one year of healthcare experience
