**Eligible for an $8,000 Sign on Bonus with two year work commitment to OSF Home Care**

POSITION SUMMARY: The LPN Home Health collaborates with the RN Case Manager to develop and implement the home health plan of care. The LPN Home Health communicates with the patientâ (tm)s provider and other members of the care team such as nursing, therapy, social work, home care aide, respiratory therapy, home medical equipment and home infusion services to facilitate and expedite the best possible patient outcomes. The LPN Home Health is responsible for a scope of care that encompasses all ages from infancy through geriatric and demonstrates the knowledge and skills necessary to provide patient care that is appropriate to the ages of the patients served. The LPN Home Health provides evidence-based, patient-centered care at the patientâ (tm)s place of residence, using critical thinking and decision-making skills. The LPN Home Health has a thorough understanding of their impact on 5 Star Agency Performance and Value Based Purchasing performance.

Qualifications:

REQUIRED QUALIFICATIONS: 

Licensure/ Certification: Current American Heart Association HealthCare Provider BLS is required prior to start date of employment.

LPN License.

Valid state driver's license.

Other Skills/ Knowledge: Excellent interpersonal and communication skills.

Solid computer skills, including proficiency with Microsoft software.

Strong analytical and problem-solving skills, with the ability to be detail oriented.

Provides own transportation and auto insurance and abides by all laws, rules and recommendations for safe driving while on duty.

Maintains a valid driver's license.

 PREFERRED QUALIFICATIONS:

Education: Licensed Practical Nurse Training

Experience: Experience as an LPN in a healthcare environment, Home Health preferred

Electronic Medical Record and Microsoft application basic proficiency.

Other Skills/ Knowledge: Electronic Medical Record and Microsoft application basic proficiency
