Responsibilities:

The Care Coordination Department at St. Josephs Hospital and Medical Center plays a critical role in providing exceptional patient care to its birth to adult in-patient population. By coordinating the delivery of healthcare and social services while balancing individually identified patient and family medical and psychosocial needs with cost-effective and available resources members of the Care Coordination Team are dedicated to furthering the mission of the Hospital. 

Being dedicated to patients and family-centered care, you value the personal self-determination of your patients. To succeed in this role, you must manage multiple priorities simultaneously and engage in creative, compassionate, and ethical problem-solving.

Expectations:

* Perform comprehensive assessments and develop treatment plans that integrate the medical, social, and resource issues that impact individual patients, families, and at-risk populations
* Establish and maintain professional relationships with physicians, case managers, nursing staff, county agencies, community resources, patients, and families. 
* Serve on hospital committees and develop and present in-services to Hospital staff.

Qualifications:

Minimum: 

* 1-Year Post-MSW experience or Social Work internship in a clinical or medical setting
* LMSW license from the AZ Board of Behavioral Health Examiners 
* Master's Degree from a School of Social work accredited by the Council of Social Work Education 
* Ability to work independently as needed with a high degree of detail orientation
* Ability to work efficiently in a fast-paced environment with changing priorities 
* Ability to work collaboratively with an interdisciplinary care team

Preferred:

* Minimum 3-Year Post-MSW healthcare experience
