The Hospice Clinical Pharmacist will support the operations of OnePoint Patient Care's clinicalteam in our East Region and will report to the Vice President of Clinical Services.

Shift: Monday-Friday 10:00am-6:00pm, possible weekend rotation and on call

Responsibilities:

* Works with Pharmacy Director to implement work flow and productivity measures that will achieve performance targets and customer expectations
* Works with Pharmacy Director and pharmacy staff to execute Company Initiatives, Core Processes and Quality Initiatives to continually improve the overall quality of service to our customers
* Works with Pharmacy Director and pharmacy staff to ensure the business unit is in compliance with all Federal, State and Local laws/pharmacy regulations and Company Corporate Compliance Program
* Assist in managing pharmacy personnel issues in conjunction with Pharmacy Director
* Works with Client Services and Sales team to effectively communicate with customers (Director Of Nursing, Administrator, Staff Registered Nurse) and resolve customer issues
* Works with pharmacy management to ensure implementation of daily operational quality and cost containment strategies (pre-pack, front-end process, cycle-fill) occurs on all dispensing and procurement activities
* Works with pharmacy management to monitor the purchasing and inventory supply for the pharmacy to ensure goals are being met (inventory turns, preferred products, generics, TI, controlled drug monitoring)
* Performs other tasks as assigned

Qualifications:

Education/Learning Experience

* Required: Bachelor's degree
* Desired:  Pharm D. from an accredited School of Pharmacy

 Work Experience

* Required: Experience in pharmacy setting
* Desired:  Long-Term Care pharmacy experience

 Skills/Knowledge

* Required: Knowledge of federal and state pharmacy regulations; computer skills

 Licenses/Certifications

* Required: As government and business needs dictate
