Job Summary

The Lead Case Manager provides program coordination, operational, and frontline staff support at our Downtown young adult transitional housing facility. This role is 100% onsite and participates in an on-call rotation to provide after-hours crisis
support as needed. The role supports low-income youth and young adults (ages 18-24) who may have experience in foster care, homelessness, disconnection from school, juvenile justice, and/or other systems. The Lead Case Manager creates 
a welcoming, trauma-informed environment while helping young people develop the skills and resources needed to live independently and thrive.

This position is at the Social Impact Center Y, the social services branch of YMCA of Greater Seattle (YGS). The mission of the Social Impact Center Y is to accelerate young people's ability to build safe, successful, and happy lives. Every year, we serve 5,000 children, youth, young adults, and families across King County with programs that aim to strengthen young people's ability to develop to their fullest potential through four strategies: reducing risk factors; navigating systems towards successful life transition; ensuring stability, safety and wellness in times of crisis; and building competencies and social capital. The Social Impact Center Y's programs include housing, behavioral health, foster care licensing, violence prevention, education, and employment. Many of the young people we serve have had involvement in the foster care, criminal justice system, and/or homelessness systems.

Hiring Range: $28.00 - 31.00/hr
Schedule: Onsite Tuesday-Saturday 2pm-10pm

Responsibilities:

1.) Leads program coordination across one or more program areas, ensuring adequate staff coverage, consistent quality of service, and smooth day-to-day operations. Serves as primary operational point of contact in the absence of the 
Program Director.
2.) Provides direct support to staff through regular coaching meetings, case consultations, and real-time feedback on practice. Serves in a leadership capacity, providing peer support, mentorship, and informal guidance to case managers,
volunteers, interns, and/or AmeriCorps members; models trauma-informed, equity-centered practice.
3.) Carries an active caseload providing specialized case management services within a transitional housing setting, addressing housing stability, education, workforce development, and independent living skills; supports young adults in
setting and achieving self-identified goals across key life domains.
4.) Develops individualized, strengths-based, youth-driven service plans in collaboration with participants; conducts regular reviews and updates to reflect progress, emerging needs, and changing goals.
5.) Builds trust-based, trauma-informed relationships with participants using a strengths-based, young adult-centered approach; maintains professional boundaries while fostering a welcoming, affirming, and culturally responsive environment.
6.) Provides crisis assessment, de-escalation support, and safety planning as needed; connects participants to behavioral health or community resources and participates in an on-call rotation for after-hours crisis support.
7.) Partners with Program Director to monitor contract compliance, track outcomes, and prepare funder reports; ensures staff understanding of grant terms and conditions.
8.) Maintains clear, professional, and timely contact notes, service plans, progress updates, and outcome data in designated databases; reviews and audits team documentation to support data quality and funder reporting.
9.) Coordinates and oversees delivery of life skills curriculum, job readiness programming, and workshops individually and in groups. Collaborates with YMCA staff and community partners to plan and facilitate activities; assesses gaps and 
adapts curriculum to meet residents' evolving needs.
10.) Builds and maintains relationships with funding liaisons, service providers, businesses, and educational institutions; represents the program at community meetings, coalitions, and interagency workgroups as assigned.
11.) Monitors participant progress across key domains - safe and stable housing, permanent connections to supportive adults, education and employment, health and well-being, and life skills - identifying barriers and adjusting services 
accordingly.
12.) Leads program intake, eligibility screening, and waitlist coordination; oversees move-in and move-out processes including housing readiness assessments and transition planning to permanent housing.
13.) Participates in the YMCA annual fundraising campaign to engage community members in volunteerism and philanthropic support for the Y mission.

Qualifications:

* Bachelor's degree in psychology, social work, administration or related field or equivalent experience, Masters preferred. Other applicable education, training, and experience, which provide the knowledge, abilities, and skills necessary to perform effectively in the position will be considered.
* Minimum three years of experience working with teens or young adults in foster care, group care, housing, education, juvenile justice, anti-poverty, vocational support, mental health, chemical dependency and/or homelessness fields. 
* Ability to teach effective independence skills for a variety of learning styles and to plan supportive activities.
* Strong youth engagement and relationship-building skills.
* Strong verbal and written communication skills, including skills in coaching and giving/receiving feedback.
* Strong organizational skills.
* Basic computer skills and working knowledge of Microsoft Office.
* Current state approved First Aid and CPR certification.*
* Current approved HIV blood borne pathogens training.*
* Prefer knowledge of and previous experience with, diverse populations (language, culture, race, physical ability, sexual orientation, etc.). Ability to speak any language in addition to English may be helpful. 

*       Within 30 days of employment or first available training.

Additional Preferred Qualifications:

* Minimum one year of experience supervising employees or full-time interns, volunteers or national service members (AmeriCorps, etc.).

This role has the potential to transport clients as an "authorized driver" for the YMCA of Greater Seattle (YGS) using your own vehicle (proof of current auto insurance and vehicle registration are required).
