Responsibilities: * Provide direct patient care to hospice patients, including appropriate pain/symptom management * Implement nursing practice following a comprehensive assessment and plan of care * Provide accurate and timely documentation of patient services * Assess and provide patient and family/caregiver education * Participate in coordination of hospice services * Provide appropriate pain/symptom management and evaluate patient response to treatments/medications * Participate in professional growth and development opportunities and quality assessment activities * Ensure compliance with all state and federal legal and regulatory requirements * Additional responsibilities as assigned Qualifications: * Graduate of an accredited practical or vocational nursing program with current licensure to practice in state of operation * Three years nursing experience. Community health/hospice or medical/surgical experience is preferred * Knowledge of the hospice philosophy of care * Commitment to clinical and documentation excellence * Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. * Possess and maintain current CPR certification.
