Responsibilities:
The Staff LVN is an experienced practitioner who demonstrates in-depth knowledge of nursing, patient care, and the ability to practice independently within the confines of educational preparation, physician orders, and as assigned and supervised by a registered nurse. this position works in the field making home visits to hospice patients in their homes, in skilled nursing facilities and / or in assisted living facilities.
Qualifications:
Minimum Requirements:
* At least two years of acute hospital experience OR Skilled Nursing experience
* California Licensed Vocational Nurse license (CA LVN)
* Current and valid CPR-AHA (CT CPRBLS)
* Current Car Insurance with proof of liability insurance
* Current California Driver License with clean driving record (CA DL)
Preferred Requirements:
* Hospice or home health experience
* SNF experience
* Experience with patients at end of lice
Special Skills:
* Demonstrates ability to work in the home setting, in SNF or ALF setting
* Works with an interdisciplinary team, under the direction of an RN
* Demonstrates proficient intermediate nursing skills
* Demonstrates a caring, compassionate attitude towards others
* Ability and willingness to actively participate in agency task forces and committees
* Demonstrates ability to organize and prioritize work efficiently
* Demonstrate clear, effective written and oral English skills, and objective, active listening skills
* Demonstrates ability to work on the computer, as documentation is computer based
* Reacts calmly and efficiently in emerencies
* Excellent interpersonal skills and the ability to work with others across all agency departments
