Responsibilities: The Staff LVN is a practitioner who demonstrates in-depth knowledge of nursing patient care and the ability to practice independently within the confines of educational preparation physician orders and as assigned and supervised by a registered nurse. * Demonstrates basic technical nursing skills within scope of practice (as documented in agency skills checklist). * Utilizes the nursing process to provide safe, therapeutic care for patients: Assessment Planning in collaboration with RN Implementation Evaluation * Documentation of the nursing process reflects pro-ficient level of clinical practice. * Implements established plan of care, follows frequency, works toward established goals and reports any necessary changes. * Interacts with Staff. * Collaborates, cooperates and coordinates with RN Case Manager/supervisor to assure effective, consistent management of clients. Works as a collaborative member of the treatment team and reports any changes in client status to case manager/supervisor in a timely manner. Qualifications: Required * High School Graduate OR High School GED * At least one year of acute hospital experience * Licensed Vocational Nurse: CA (LVN:CA) * Basic Life Support - CPR (BLS-CPR) * Driver's License: CA (AUTO:CA) * Auto Insurance (AUTO-INS) Preferred * Home health experience
