Responsibilities:

The Staff LVN is an experienced practitioner who demonstrates in-depth knowledge of nursing, patient care, and the ability to practice independently within the confines of educational preparation, physician orders, and as assigned and supervised by a registered nurse.

* Demonstrates basic technical nursing skills within scope of practice (as documented in agency skills checklist).
* Utilizes the nursing process to provide safe, therapeutic care for patients: Assessment, Planning in collaboration with RN, Implementation, Evaluation.
* Documentation of the nursing process reflects pro-ficient level of clinical practice.
* Interacts with Staff Collaborates, cooperates and coordinates with RN Case Manager/supervisor to assure effective, consistent management of clients.
* Documents services rendered and client response. Documentation is complete, accurate, timely and legible.
* Complies with safety, infection control, CHW Integrity Program, and other organizational policies (90-day evals- department specific orientation checklists completed; annual evals -annual competency checklists completed).

Qualifications:

Required

* High School Graduate and At least one year of acute hospital experience OR High School GED
* Licensed Vocational Nurse: CA (LVN:CA)
* Basic Life Support - CPR (BLS-CPR)
* Driver's License: CA (AUTO:CA)
* Auto Insurance (AUTO-INS)

Preferred

* Home health experience
