Responsibilities:

As an LPN you will:

* Perform patient observation, data collection related to patient care, administers medication, treatment and procedures according to the physicians' orders and resident care plan. 
* Communicates patient findings to the supervisor. 
* Accountability for facilitating, communicating and collaborating with the healthcare team to identify and meet the physical, emotional and spiritual needs of the patients.
* Oversee care provided by CNAs, Care Partners, QMAPs and/or other support staff, while also promoting the optimal health, well-being and safety of the patient through use of the nursing process and in accordance with patient care standards, guidelines and the State Nurse Practice Act. 
* Demonstrate personal accountability for relationship-based care, organizational mission, and core values. 

Qualifications:

In addition to bringing your whole self to the workplace each day, qualified candidates will need the following:  

* One year of previous LPN experience, preferred
* Requires knowledge in nursing and person-centered care concepts
* Ability to demonstrate competency in the provision of individual care of patients based on the patients' assessed age and developmental needs.
* HS GED
* Current state LPN license for applicable state of employment or current LPN license from a participating state in National Compact Licensure (Colorado only)
* BLS

Medium Work - (Exert up to 50lbs force occasionally, and/or up to
20lbs frequently, and/or up to 10lbs constantly)/

The territory that this PRN LPN will cover will be the northern Metro area.
