As an LPN in this care model, you will provide care for a specified group of patients, utilizing all the skills you have gained in obtaining your license. Recognized as a valuable member of the Acute Care team, the LPN practices under the supervision of the Registered Nurse and assists patients, while providing a therapeutic environment for patients and their families. Actions are team oriented to successfully provided patient/family care and high level of satisfaction, through evidence- based practice. Maintains a practice environment that reflects the ANA Code for Nurses, Philosophy of Nursing at UnityPoint Health within the structure of the Professional Nurse Practice Model and in collaboration with other care givers. Responsibilities: Clinical Excellence & Patient Care Possesses the knowledge and clinical skills necessary to provide care for people with physical disability and chronic illness Performs hands-on nursing care by utilizing the nursing process to achieve quality outcomes for patients Reinforces the care provided by other team members Provides direction and supervision of unlicensed assistive personnel (i.e. Patient Care Technician), demonstrates clinical judgment appropriate for Iowa's LPN scope of practice, uses problem solving techniques and time-management principles, and delegates appropriately Demonstrates effective oral and written communication skills to develop a rapport with patients, their families, and health team members and to ensure the fulfilment of requirements for legal documentation and reimbursement Contributes to the assessment of the physical, psychological, sociocultural, and spiritual dimensions of patients and their families, as well as their educational and discharge needs Contributes to the evaluation of the nursing care and to the modification of the plan as needed to achieve measurable goals and objectives Intervenes with team members and other healthcare professionals to ensure that the optimal opportunity for recovery is made available to the patient. Collaborates with the staff nurse and other team members to achieve cost-effective care by utilizing appropriate clinical measures to meet emergent physical, psychosocial, and spiritual situations Communicates effectively and interacts in a manner that facilitates the group process and team building. Offers input into team decision making within their scope of practice. Advocates and supports the rights of patients and families with care decisions. Intervenes on behalf of patients, along with the staff nurse, to ensure that medical and nonmedical professionals work to maximize patients' success at time of discharge and beyond. Practice reflects knowledge of federal/state regulatory and accrediting agencies, i.e. Commission on the Accreditation of Rehabilitation Facilities (CARF) and the Iowa Department of Inspection and Appeals (DIA). Performance Improvement Focus: Actively participates in data collection, analysis, problem solving and gives input on process improvements and patient satisfaction. Assures that the care provided is consistent with standards of care, evidence-based practices, national patient safety goals, core measures and other quality initiatives undertaken by the unit/hospital Works with interdisciplinary team in identifying opportunities for improvement in patient care and patient/family satisfaction Actively promotes and achieves customer satisfaction (patient, family, departments, providers) Participates in the implementation of Performance Improvement initiatives Promptly and efficiently follows up on patient/family requests/needs - anticipates patient/family needs Encourages coworkers in achieving excellent customer satisfaction; utilizes organizational service excellence standards Identifies practice problems on the unit with solutions generated.
