Elderwood at Ticonderoga: 101 Adirondack Dr., Suite 3, Ticonderoga, New York 12883 Responsibilities: What You'll Do (LPN - Licensed Practical Nurse) * Administer medications and treatments safely and accurately (following the "5 rights") * Lead and support CNAs in delivering high-quality resident care * Monitor resident conditions and report changes to nursing leadership * Follow and implement care plans to ensure consistent, individualized care * Maintain accurate clinical documentation (MARs, treatment records, nursing notes) * Ensure safe handling of medications, including controlled substances Qualifications: What We're Looking For (LPN - Licensed Practical Nurse) * Current LPN (Licensed Practical Nurse) or RN (Registered Nurse) license in New York State * Experience in long-term care, skilled nursing, or geriatric settings preferred * Strong ability to lead and support CNAs and care teams * Clear communication skills - both verbal and written * A compassionate, respectful approach to resident care * Ability to work in a fast-paced, hands-on environment * Commitment to delivering care aligned with high clinical and regulatory standards
