How YOU will benefit * Provide 1:1 care to make a lasting impact on patients and families * Greater work/life balance with flexible scheduling options * Less time on your feet compared to other settings * Ability to work independently while also having team support * Job stability and regular advancement opportunities with a growing company As a Hospice LPN You will: * Give hands-on care to patients nearing the end of life, following state nursing rules. * Follow the care plan after assessing the patient's needs. * Keep clear and timely records of the care provided. * Teach and support patients and their families about the illness and care plan. * Help coordinate care by communicating with the hospice team (like aides, chaplains, social workers). * Manage pain and symptoms, checking how well treatments are working. * Take part in training and quality improvement activities to grow professionally and improve care. Community health/hospice or medical/surgical experience is preferred. * Currently licensed as an LPN/LVN in the State. * Comply with accepted professional standards and practice. * Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. * Possess and maintain current CPR certification. * Understand philosophy of hospice concept. Also understand needs of the terminally ill. * Use own knowledge and experience base and other resources as necessary to make logical decisions and solve problems. * Demonstrate good verbal and written communication, and organization skills.
