LPN Coach - Care Connections (Fulltime, Remote)

Location: Remote (Preferred candidates in Charlotte or Triad area) 

Schedule: Monday - Friday | 8:00 AM - 4:30 PMTraining: Up to 3 weeks of onsite training required prior to remote transition (Charlotte and Winston-Salem training sites available)

Help Patients Stay on Track Post-discharge
At Novant Health, our LPN Coaches play a critical role in supporting patients after discharge and helping them successfully navigate the next steps in their care. In this telephonic, virtual care setting, you'll connect with patients, coordinate follow-up services, and ensure they have the resources they need to stay healthy and avoid unnecessary hospital readmissions.

As part of the Care Connections team, you'll collaborate with physicians, advanced practice providers, clinic staff, and other healthcare professionals to deliver seamless care across the healthcare continuum.

Your outreach, clinical insight, and patient-centered communication help ensure patients feel supported long after they leave the hospital.

What You'll Do
* Conduct post-discharge follow-up calls to assess patient needs and ensure successful care transitions

* Schedule follow-up appointments with primary care providers and specialty clinics

* Support Medicare Shared Savings Program (MSSP) patients with preventive care outreach, including annual screenings and wellness requirements

* Conduct virtual Annual Wellness Visits with eligible patients

* Coordinate patient outreach and communication with Skilled Nursing Facilities (SNFs) and other post-acute partners

* Document patient interactions and care coordination activities in the electronic medical record

* Collaborate with physicians, advanced practice providers, and care teams to ensure appropriate follow-up and continuity of care

What We're Looking For
* EPIC experience, highly preferred.

* Experience in patient services or an ambulatory clinic setting, highly preferred.

* Education: High School Diploma or GED, required. 2 Year / Associate Degree, preferred. 
* Experience: Minimum three years clinical experience in Hospital, Home Health, or community setting, required. Recent focus on acute and/or chronic disease management, preferred. 
* Licensure/Certification: Current, unrestricted LPN license for the states in the NH footprint, required. 
* Additional Skills (required): Broad clinical knowledge base of multiple conditions and expected outcomes. Working knowledge of effective teaching techniques, applying adult learning principles. Ability to demonstrate coordination of appropriate educational materials for patients and their support systems. Knowledge of good business English, Spelling, and Math. Demonstrates appropriate professional nursing skills in the provision of preventive health maintenance and/or treatment of illness. Demonstrates a working knowledge of Novant Health and the physician office setting to act with providing connections across the system. Demonstrate appropriate clinical leadership skills as evidenced by previous work related activities and performances. Demonstrate the ability to work under the direction of the RN to assess and evaluate understanding of disease processes, treatment plans and/or lifestyle changes. Demonstrate appropriate communication, negotiating and networking skills. Demonstrate sound knowledge and appropriate decision-making skills for designated patient populations. Demonstrate appropriate organization skills. Demonstrate the ability to provide program specific outcomes. Demonstrate the ability to assess self learning needs and seek ways to meet own professional development by proactively educating self, participating in orientation and staff development activities, maintaining knowledge of clinical practice, issues, trends, technology and available community resources, and uses policies and procedures appropriates. Possesses excellent computer skills with the ability to multitask including navigating and documenting in an EMR while communicating with the caller and using a telephonic platform.
