Responsibilities:

Are you a compassionate and skilled Social Worker looking to impact lives? Join our dynamic Care Coordination team and provide vital support to patients and families in our hospital and emergency department.

In this role, you will:

* Provide crisis intervention, psychosocial assessments, and counseling.
* Advocate for patients and families, connecting them with essential resources.
* Facilitate complex discharges and transitions of care.
* Collaborate with an interdisciplinary team to ensure optimal patient outcomes.

Schedule: Friday - Monday 7am - 5:30pm

Student loan repayment program as well as relocation assistance offered!

Qualifications:

We're looking for someone with:

* Master's degree in Social Work (CSWE accredited)
* 2+ years post-MSW experience (5+ years preferred)
* Oregon LCSW or CSWA working towards licensure (LCSW required within 2 years)
* Excellent communication, problem-solving, and teamwork skills
