Intake/Home Care Coordinator

Full Time - 1.0 FTE

Shift - 8:30 AM - 5:00 PM

Responsibilities:

* Coordinate In-Home Care Services - Schedule initial home care appointments, arrange equipment delivery, and obtain insurance authorizations. Utilize the home care coordination team to ensure seamless service for patients and referrals.

* Manage Patient Intake & Communication - Process incoming referrals, gather demographic and financial information, verify insurance benefits, and direct inquiries to the appropriate service lines. Maintain clear and professional communication with patients, referral sources, and third-party payers.

* Ensure Compliance & Customer Satisfaction - Maintain knowledge of regulations, follow up with patients, and provide excellent customer service. Assist with team coordination as needed, participate in on-call rotations, and uphold UnityPoint at Home's reputation as a high-quality care provider.

Qualifications:

* One year medical experience/previous health care or home care related Intake/Coding + customer service experience
* High-school diploma
