Overview: At CHI Health CUMC-Bergan Mercy, our focus is patient-centered care. Our level I trauma center and academic medical center campus is designed to heal the body, mind and spirit of every person in a more comfortable, less stressful hospital environment. Our full range of medical services includes trauma services, heart and vascular care, emergency services, surgery, maternity, cancer care and diagnostic imaging. Responsibilities: * Compounds and dispenses medications. Interprets medication orders and/or prescriptions for drug interactions, appropriate drug selection and appropriateness. Accurately enters the order or prescription into computer system, labels, and dispenses medications in a timely manner. * Demonstrates ability to document clinical activities and interventions such as consults, patient teachings and other patient specific information as defined by policy and procedure. * Verifies and checks medications prepared by other pharmacy personnel. * Communicates drug information to patients and health professionals. Resolves drug related problems by consulting with other health care professional to achieve appropriate patient outcomes. Unclear orders written by a physician are clarified with a physician. Assists the physician in appropriate drug selection and therapeutic regimens. * Achieves and maintains an adequate basic knowledge base to provide clinical services for all age groups served. Provides basic clinical services as required by policy and procedure. * Maintains a working knowledge of automated systems * Responsible for technician activities and performance. Provides direction and guidance as needed. * Participates in the orientation, instruction and training of pharmacy students and interns, pharmacy technicians, residents, pharmacists and other health professionals. * Performs duties in documented accordance with requirements of all regulatory and governing bodies. Mandatory education from a system and department level is completed. * Patient counseling and drug information and documentation of such. nepharm Qualifications: Required Education for Staff Job Levels A B.S. or Doctor of Pharmacy degree Required Licensure and Certifications NE Pharmacist License Required Minimum Knowledge, Skills and Abilities Must possess strong knowledge of Pharmacy Services; knowledgeable and skilled in current practice concepts of nursing home, community pharmacy service, and home I.V. Therapy; strong knowledge of applicable laws, regulations, guidelines and professional standards; strong project management skills; good strategic planning skills; strong change management skills; must demonstrate personal traits of a high level of motivation; team orientation; professionalism; trust; and place a high value on treating others with dignity and respect.
