Shift: Monday, Tuesday, + Friday | 7:00 AM - 3:30 PM (+ occasional holidays/weekends)

As a Hospice RN with UnityPoint at Home, you'll provide one-on-one care to patients with total autonomy over your role. With a flexible schedule and independence, our hospice nurses are in charge of everything from patient admission to the care plan and make it possible for patients to receive care where they're most comfortable - in their homes. 

The Hospice RN uses clinical nursing knowledge, physical assessment, teaching and procedural skills to deliver high quality patient care in the patient's place of residence. The Hospice RN develops and oversees the health plan of care under the direction of the physician and ensure appropriate interdisciplinary involvement to ensure the best outcome for each patient. The Hospice RN delivers patient care directed by the physician as established in the home health plan of care that is consistent with clinical best practices and results in high quality, improved outcomes and exceptional patient experience.

Responsibilities:

  Makes visits to the homes of patients requiring hospice nursing services and assumes responsibility for an ongoing interdisciplinary assessment of the patient.

  Ensures compassionate, empathetic quality and safe delivery of services to patients according to care plan. Uses equipment and supplies safely, effectively, and efficiently.

  Consistently utilizes a patient-centered approach considering physical, psych/social, spiritual, educational, safety and related criteria appropriate to the age of the patients served in hospice.

  Delivers quality care and completes clear, concise and accurate clinical documentation of care provision in accordance with hospice conditions of participation (CoPs), various payer hospice coverage criteria, accreditation standards, professional standards for discipline/credential and the hospice plan of care.

  Maintains accurate and timely documentation of clinical records and time/travel entries

Qualifications:

Education:

* Graduate of State Board approved program for Registered Nurses

Licensure/Certifciations:

* Valid license as Registered Nurse in state where providing care.
* Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements.
* Mandatory Reporter: Child & Dependent Adult Abuse.
* CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.

Minimum Experience:

* One year nursing experience caring for similar patient population as to be assigned.

Other:

* Use of usual and customary equipment used to perform essential functions of the position.
