Hospice Liaison Nurse - Full-Time, Days 

Charlotte Hospice - Charlotte, NC

Schedule: 8:00am - 5:00pm, Day Shift

This position may be eligible for a sign-on bonus and relocation.

In this meaningful role, you'll clinically assess and admit patients to hospice services, coordinate discharge planning, and collaborate closely with families, acute care teams, and healthcare partners to ensure seamless, compassionate transitions.

In this role, you will:

*  Assess and admit patients to hospice care from hospitals, clinics, or other settings

*  Facilitate discharge planning and coordination with inpatient teams and facilities

*  Serve as a bridge between the care team and patients' loved ones

*  Advocate for patient comfort, preferences, and quality of life

Qualifications:

* Education: High School Diploma or GED, required. 4 Year / Bachelors Degree, preferred. Graduate of an accredited school of nursing required; BSN preferred.
* Experience:  Minimum three years' experience in home care or hospice, required.  Previous experience in managed care, infusion care, and case management and previous home health/hospice case management experience, preferred.
* Licensure/Certification: Current RN licensure in appropriate state, required.
* Additional Skills (required):
* Ability to provide direct patient care, demonstrate excellent nursing skills
* Excellent verbal and written communication skills, problem solving skills, and basic math skills.
* Understands hospice philosophy and issues of death/dying and demonstrates competency in pain and symptom management with EOL care.
* Ability to interpret information to identify each patient's requirements for care relative to his/her age specific needs.
* Ability to prioritize, organize and handle varying and unpredictable situation such as emergencies or crisis situations.
* Must be flexible, versatile, adaptable to change, with good critical thinking, problem solving and diplomacy skills.
* Knowledge of procedures and nursing standards of practice, both general and specific to the specialties of medical- surgical nursing.
* Ability to interact effectively with diverse individuals at all levels of authority, in teams and on committees.
* Actively identifies new areas for learning; regularly creating and taking advantage of learning opportunities; using newly gained knowledge and skill on the job and learning through their application.
* Must have computer experience and have reliable transportation to and from all work related assignments and activities.
* Must be flexible to travel as needed between different locations, as assigned, in all weather conditions and on roads in varying degrees of repair.
* Must be available for after hours call on rotational basis.
* Specific vision abilities required by this job include close vision, distance vision, night driving, and ability to adjust and focus.
