Overview:
Location: Charlotte Hospice
Shift: 8-6:30pm, 4x10hrs - Tues. - Fri.
Responsible for managing the day-to-day clinical activities as assigned by the Supervisor/Manager. Responsibilities include direct patient care, coordination and implementation of care, scheduling and resource management, and indirect supervision of hospice interdisciplinary team.
Qualifications:
* Education: 2 Year / Associate Degree, required. Graduate of an accredited school of nursing required; BSN preferred. 4 Year / Bachelors Degree, preferred.
* Experience: Minimum three years' experience in hospice or home health care, required. Experience in direct care of patients with life limiting illnesses strongly preferred. Related experience include oncology; family medicine; internal medicine; geriatrics, medical surgical, and/or case management.
* Licensure/certification/registration: Current RN licensure in appropriate state required. Current Basic Life Support for Healthcare Provider (BLS), required. CHPN/CHPCA, preferred.
* Additional Skills (required):
* Ability to provide direct patient care, demonstrate excellent nursing skills, judgment, and understanding of Hospice philosophy.
Excellent verbal and written communication skills, and problem-solving skills
Ability to prioritize, organize and handle varying and unpredictable situation such as emergencies or crisis situations.
Ability to interpret information to identify each patient's requirements for care relative to his/her age specific needs.
Must be flexible, adaptable to change and able to apply problem-solving skills and critical thinking skills.
Ability to interact effectively with diverse individuals at all levels of authority, in teams and on committees.
Must have knowledge of nursing and pharmacy procedures and standards of practice, both general and specific, to the specialties of infusion and enteral therapies.
Must have computer experience and have reliable transportation to and from all work related assignments and activities.
Must be flexible to travel as needed between different locations, as assigned, in all weather conditions and on roads in varying degrees of repair.
Ability to work in environment containing distractions and interruptions such as noise and telephone calls.
Must be available for after hours call on rotational basis, as assigned.
* Additional Skills (preferred):
* Knowledge of Hospice services necessary to obtain and interpret information in terms of the patient's needs in order to appropriately receive referrals.
Knowledge of Medicare/Medicaid rules/regulations necessary to understand criteria for determining patient eligibility for admission.
Ability to interpret insurance agreements/managed care contracts to provide appropriate care to variety of patients.
Ability to effectively negotiate contract/non-contracted rates for appropriate managed care depending on fair market analysis of current pricing trends.
