				Overview:

Provides comprehensive community home visiting services to women and their families as part of the Healthy Families America (HFA) program. Adheres to the HFA model, and to policies, procedures, guidelines and standards of HFA, The Center for Family Safety and Healing, and Nationwide Children's Hospital. Maintains professional boundaries and provides excellent service and in-home support to expectant mothers, their children, and their families.

Schedule: Monday - Friday 8:00am-5:00pm

Responsibilities:

*  Provides general child and family advocacy, case management, and evidence-based prevention support to individuals who present to The Center for Family Safety and Healing within the Healthy Families America Home Visitation Program.

*  Meets with families in their homes, identified community setting, or virtually online via telehealth; delivers services, provides parenting education, offers support and problem-solving skills daily. Develops family goal plans in collaboration with families that incorporate the families' strengths, visions, goals, and priorities.

*  Educates families as to expected developmental milestones for their child as well as the importance of routine medical and health services. Links parents and children to appropriate resources within Franklin County. Understands and implements family-centered, linguistically, and culturally appropriate approaches in the family's everyday routines and activities. Provides services which are culturally sensitive to the families being served.

*  Provides services within the community and follows all hospital policies regarding off-site work. Maintains sound working knowledge of community resources.

*  Completes all required documentation and paperwork requirements in a competent, legible, and timely manner consistent with legal and agency policy or program mandates. Responds to individualized family needs and situations as required and consistent with departmental expectations.

*  Completes routine screening with families using programmatic tools for monitoring of child development, perinatal depression, environmental safety, family violence, and adverse childhood experiences.

Qualifications:

Education: Bachelor's degree in social work, human ecology, child development, education, nursing, or a related field, required.

Licensure: LSW or RN preferred but not required.

Skills:

*  General knowledge of child and family development and related concepts

*  General knowledge of community resources and social supports in the Franklin county and surrounding areas.

*  Computer system proficiency such as Outlook, Word, etc.
