Overview: $1,500.00 New Hire Sign-On Bonus! Shift: Monday - Friday | 8:00 AM - 5:00 PM (+ rotating weekends) As a Home Care/Hospice Aide, you are a vital part of our compassionate care team, working closely with the RN Case Manager to provide personal care, light homemaking, and heartfelt companionship to patients in home/hospice care. Your role is more than just meeting needs-it's about creating comfort, preserving dignity, and bringing moments of joy during life's most tender stages. Responsibilities: Uses appropriate and safe techniques in performing personal hygiene and grooming tasks such as bed or tub bath, shower assistance, hair care, oral hygiene, toileting and incontinence care, light housekeeping, and other tasks assigned by the RN case manager Creates successful interpersonal relationships with patients/families while maintaining personal and professional boundaries Reports changes in the patient's medical, nursing, and social needs to the RN case manager as the changes relate to the plan of care Completes rover care plan accurately and in a timely manner while being compliant with the hospice program policies and procedures Demonstrates the UnityPoint Health Values and Standards of Behaviors as well as adheres to policies and procedures and safety guidelines. Qualifications: Education: * Completion of a Nursing Assistant Training Program of a minimum of 75 hours with no continuous period of 24 months without compensation for Home Care Aide/CNA work since completion Licensure/Certifications: * Valid driver's license when driving any vehicle for work-related reasons. * CPR Certification * Valid Mandatory Reporter course completion by state(s) requirement * Valid automobile insurance in accordance with state and/or organizational requirements Apply Today! * Ready to take the next step in your healthcare career?
