Responsibilities:

Under the direction of the Program coordinator and in collaboration with the multi-disciplinary team, the Navigator serves as a clinical resource in providing direct and indirect care for patients aimed at optimizing patient assessment, continuity of care, education to patients and family members and facilitates on-going treatment.  Supports data collection for appropriate TVT Registry.  Serves as a resource and contact for program to facilitate the collaboration and coordination of patient care services to provide the highest level of care possible.  Primary role is that of an expert nurse with essential education, consultation and researcher role.  

Qualifications:

Required Education and Experience 

Associate or Bachelor's degree

Graduate of an accredited nursing program

Required Licensure and Certifications

Nursing license 

BLS

ACLS

Required Minimum Knowledge, Skills, Abilities and Training

BSN or Master's Degree preferred.  Certification in specialty or related area preferred.
