The Healthstart Care Coordinator is responsible for providing the highest level of care coordination for the PE Medicaid process for uninsured and underinsured OB patients. The Healthstart Care Coordinator supports and implements the philosophy, goals and standards of HMH JFK University Medical Center and of the Family Medicine Center as well as those of the Healthstart Program. Works collaboratively with office Social Worker and team members to achieve shared goals and with consistent focus on quality care. Responsibilities: A day in the life of a Healthstart Care Coordinator at Hackensack Meridian Health includes: * PE MEDICAID - Complete Initial process, Second Process and Third Process. * Monitors and Reports on Amerigroup Incentive Program. * Ensures PE Application Processing is Completed. * Other duties and/or projects as assigned. * Adheres to HMH Organizational competencies and standards of behavior. Qualifications: Education, Knowledge, Skills and Abilities Required: * High School diploma, general equivalency diploma (GED), and/or GED equivalent programs. * Minimum of 2 or more years of experience in Care Coordination. * Able to multitask and work in a team. * Excellent written and verbal communication skills. * Proficient computer skills that may include but are not limited to Microsoft Office and/or Google Suite platforms.
