Position Summary 

The HIV Clinical Service Delivery Advisor will primarily support the Adult Clinical Branch in the Prevention Care & Treatment (PCT) division in the Office of HIV/AIDS within USAID's Bureau for Global Health as a Credence Institutional Support Contractor (ISC).

The U.S. Agency for International Development (USAID) is the world's largest bilateral donor in the health sector.  USAID's global health programs save lives, protect communities from deadly diseases, and promote social and economic progress. Key areas of work include family planning and reproductive health, HIV/AIDS, maternal and child health, nutrition, malaria, tuberculosis, neglected tropical diseases, innovation and impact, and health systems strengthening.

The Bureau for Global Health's Office of HIV/AIDS (GH/OHA) is the center of HIV/AIDS technical leadership for USAID and has primary responsibility for the Agency's HIV/AIDS program. OHA provides strategic direction, technical leadership, and support to field programs in HIV/AIDS.

The HIV Clinical Service Delivery Advisor supports the planning, development, implementation, and monitoring of global and country-level HIV treatment and clinical service delivery policies, strategies, and programs. 

The HIV Clinical Service Delivery Advisor will provide technical support in areas of HIV care and treatment in support of USAID's implementation of the President's Emergency Plan for AIDs Relief (PEPFAR).  The HIV Clinical Service Delivery Advisor will support the Adult Clinical Branch in the Prevention Care & Treatment (PCT) division in the Office of HIV/AIDS within USAID's Bureau for Global Health. Technical supervision and programmatic guidance for this position will be provided by the Adult Clinical Branch Chief in the Prevention Care & Treatment Division. 

The advisor will provide advisory support on issues of HIV care and treatment for adults and adolescents and build strong linkages to all aspects of the clinical cascade through intra-office collaboration and support to USAID field missions. Technical support will include a wide range of clinical programming, including HIV treatment regimen optimization; HIV laboratory monitoring protocols and strategies; differentiated service delivery (DSD) models; provision of clinical services in non-permissive environments (NPEs); supportive community-based HIV programming; and the delivery of appropriate clinical care services to reduce morbidity and mortality, such as cervical cancer, mental health and advanced disease programming. The Advisor will participate in the review and evaluation of existing program guidance documents, standard operating procedures (SOPs) and policies on HIV clinical service provision, providing recommendations for enhancing the effectiveness and/or efficiency of USAID-supported HIV treatment programs, and will work closely with headquarters and field-based counterparts to optimize continuity in care and virologic suppression for PLHIV.

Responsibilities:

* Provide USAID technical and programmatic advice on the planning, design and implementation of HIV care and treatment service delivery programs, for a broad swath of activities including:
* Clinical Service Delivery models, including differentiated service delivery (DSD) models, multimonth dispensing (MMD) and models to integrate HIV service delivery with other primary health care platforms
* HIV service delivery in Non-Permissive Environments and with complex emergencies.
* Appropriate and optimized HIV treatment regimens
* HIV laboratory monitoring protocols and strategies, particularly viral load, targeted CD4 testing, and HIV drug resistance testing
* Alignment of community-based programming to improve outcomes for PLHIV
* The delivery of appropriate clinical care services to reduce morbidity and mortality, such as cervical cancer, TB, HCV or NCD co-infection treatment, mental health and advanced disease programming
* The integration of technology (such as machine learning, e-health and generative artificial intelligence) into existing clinical or public health programs

* Participate in USAID and interagency technical working groups and collaborate with technical experts and leaders with Centers for Disease Control (CDC), the Department of Defense (DoD) Peace Corps, and the State Department Bureau of Global Health Security and Diplomacy (GHSD).
* Serve as a Technical Liaison to AORs and CORs responsible for managerial and technical oversight of specific clinical services within cooperative agreements and contracts. Provide recommendations regarding the activity focus and performance of USAID implementing partners to reach 95-95-95 and sustainability goals. 
* Develop analyses or analytical tools to better evaluate program progress in HIV care & treatment and understand bottlenecks to achieving 95-95-95 targets.
* Provide support to ensure efficient use of resources and align service delivery programming with sustainability roadmaps and strategies. 
* Support field teams to prepare for quarterly POART reviews, Country Operational Plans (COPs) and other planning activities pertaining to program performance.
* Assess and identify health system-level bottlenecks that hamper continued patient adherence and virologic suppression; work collaboratively with USAID and external stakeholders to strategize solutions to be reflected in USAID programming.
* Facilitate sharing of best practices across USG programs in diverse countries
* Support OHA technical clusters and interagency technical working groups in core technical areas, such as HIV Care & Treatment, DSD, Complex Operating Environment programming, Cervical Cancer, and Community-Based Programming. Contribute technical updates, presentations, reference materials, briefers and other documents that describe USAID's activities, research, and progress; prepare portfolio reviews and reports; review documents such as annual reports and operational plans; and prepare responses to Q&As and other requests for information.
* Support the evolution of HIV treatment programming to safely and effectively integrate new technology that improves client/health care worker experience and outcomes. 
* Remain abreast of the latest scientific literature, reports and international guidelines and standards to ensure USAID programming reflects state-of-the-art public health approaches; prepare and submit papers to professional journals and meetings.
