Responsibilities:

Under limited supervision, responsible for assembling and maintaining patients' health information in medical records and charts.  

Essential Key Job Responsibilities

* Assemble patients' health information in chart.
* Ensure all forms are accurately completed, properly identified, and signed.
* Ensure all necessary patient information is in the computer and up to date.
* Scan pertinent information into patient's EMR.
* Obtain and handle medical record document requests from outside parties, which may include subpoenas, FMLA forms, etc. for response and regulation compliance.
* Perform other duties as assigned.

Qualifications:

Education: 

* High School Diploma or equivalent required.

Required Minimum Knowledge

* 1 year related experience in a healthcare environment required. 

PREFERRED Qualifications

* Completion of medical record technology or medical terminology course preferred. 
* Knowledge of practice management and word processing software.
* Ability to perform redundant tasks with minimal error rate.
* Ability to use good judgment in the course of contact with physicians, patients, staff, and outside sources.
