				Overview:

Dignity Health-Yavapai Regional Medical Center is a not-for-profit, integrated healthcare provider that offers a broad range of inpatient and outpatient services. Responsibilities:

* Patient population includes Geriatrics, Dementia, and Palliative Care 
* Collaborate closely with interdisciplinary teams and Advance Practice Providers to enhance efficiency and care coordination to ensure comprehensive patient care
* Actively engage in patient education and shared decision-making to empower older adults and improve health outcomes
* Serve as a Geriatrics Consultant, including screening and diagnosing Dementia
* Clinic position with possibility of home visits
* Full time position, Monday through Friday
* Provider will have 4 hours of administrative time per week
* Daily patient volume: anticipated 6-10 patients per day
* Current programming includes multi-disciplinary Dementia Screening Clinic, Alzheimer's & Dementia Care Program (UCLA dissemination site), Outpatient and Inpatient Palliative Care, and GUIDE model participant 
* Opportunity to enhance and expand services in the Cunningham Center for Compassionate Aging

Qualifications:

* BE/BC in Geriatric Medicine (IM or FP), Neurology, or Hospice & Palliative Medicine (IM or FP)
* Experience or interest in working with Dementia/Alzheimer's Disease population
* Strong communication and interpersonal skills 
* Valid state medical license as MD or DO
* Valid DEA certificate
* Strong clinical skills and experience working with older adults
* Ability to work effectively in a team-based healthcare environment
* Demonstrate empathy, compassion, and respect for the unique needs of older adults and their care partners #HEC
