Overview: This position, under the supervision of the Case Manager assists foster children, foster parents, and families by providing support and crisis management both in-home and office settings. Responsibilities: * Assist in providing client and foster parent support/crisis management * Facilitation of case-related activities on behalf of the agency * Prepare and process case service forms * Supportive counseling/referrals to needed services * Stay up-to-date on all required training: YSIS and ARC Model Training * Provide in-home services and support to youth and families * Participate in the treatment and follow service plans for youth in care * Maintain proper documentation for all services delivered * Respond and provide on-call crisis support * Attend meetings surrounding youth in placement, including Multi-Disciplinary Team (MDT) meetings, Individualized Educational Planning (IEP) meetings, and on-going collaboration meetings with referral sources and community providers * Work in collaboration with the education system * Complete court summaries and attend court hearings as needed * Collaborate with all systems and individuals serving youth, including legal guardian, juvenile probation office, appointed guardian ad litem, mental health provider and school officials * Engage biological families in treatment and service delivery, when appropriate, to support reunification efforts * Maintain professional boundaries, conduct and confidentiality and other governmental laws and regulations, agency policy and licensure ethics * Ensure the safety and well-being of all youth in care by observing for signs of abuse and/or neglect * Provide services that respect each family and youth's cultural background, heritage and identity Qualifications: * High school diploma or GED * Must be 21 years old * Valid Driver's License and reliable transportation. * Minimum two years of experience working in youth services with children and families.
