Overview: Title: FAMILY TEAM CONFERENCE FACILITATOR - Child Welfare Residential Programs Reports To: Vice President, Child Welfare Residential Programs Program, Youth and Family Well-Being Division Location: Manhattan, Brooklyn and Bronx sites (w/main office at the 17th St location) Hours: 35 Hours/Week; Salary Range: 51K-57K based on post graduate revelant experience. Job Summary: The Family Team Conference (FTC) Facilitator is responsible for facilitating FTCs on a full-time basis at 3 of the GSS' Residential programs: Marian Hall (NYC), Green House (Bronx) and Euphrasian Residence (Brooklyn). FTC meetings are designed to bring all parties involved in a young person's life (Parents/Foster Parents/Potential Discharge Resources, Attorneys/Social Workers, etc.) together to discuss case planning issues related to the youth's placement and assist the group in reaching consensus about the plan. This includes required preparation with GSS Case Planners and Supervisors (GSS), planning for space and supplies needed for each meeting-whether in person or virtual, and completion of required paperwork after the meeting concludes. The Facilitator ensures these meetings run smoothly, creating a collaborative environment conducive to open communication, and remaining focused on the tenets of Permanency, Safety and Well Being. Duties of Successful Facilitator: * Prepares for conferences by meeting with Case Planners/Supervisors, reviewing last FASP and Progress notes and/or attending Permanency Team Conferences on upcoming cases for review. * Responsible for scheduling and confirming conference dates, notifying all parties, reserving space, or virtual platform, maintaining supplies and coordinating with Admin Manager at each site of meetings planned. * Develop and maintain monthly FTC calendar * Facilitates Conferences on the dates/times assigned as well as rescheduling. * Completes FTC Summary Form and distributes timely at end of each conference. * Responsible for the documentation of FTC meetings, all pertinent communication and relevant information in Connections (CNNX, the Child welfare computer system). * In collaboration with PEP (Program Evaluation and Planning) Department, attends meetings to ensure compliance with FTC model and maintains statistics for tracking and quality improvement purposes * Maintains/expands professional skills by attending ongoing training when available and required. * May represent Agency at ACS and other outside meetings relevant to the FTC initiative. * Other duties as assigned. Qualifications of Facilitator: * Bachelor's Degree (Psychology; Social Work or relevant field) with two years related experience and/or Child Welfare experience * Master's degree in social work preferred * Knowledge of Child Welfare guidelines preferred. * Ability to guide participants to consensus by facilitating vs directing * Interpersonal skills necessary to engage complex clients and many different levels of staff. * Excellent organization and time management skills. * Strong written and verbal skills necessary * Bilingual skills preferred. * Must have a commitment to work from a Trauma Informed, strength based and/or youth development perspective.
