* Collaborate with a multidisciplinary team to develop individualized care plans, coordinate referrals, and engage in community health initiatives to improve overall patient outcomes.

Candidate Qualifications:
* Board-certified or board-eligible in Internal Medicine or Family Medicine
* Graduate of an accredited school of medicine required
* Current MD license in appropriate state
* Current DEA license required
