				Overview:

Provides advocacy, interventions, support, and prevention services to families, expectant mothers, and single adults who are experiencing significant housing instability and at-risk of literal homelessness. Collaborates with the Stable Housing Network to rapidly resolve the immediate housing crisis and connect clients with appropriate resources.

Responsibilities:

*  Utilizes risk screening tools to assess housing needs and crises. 

*  Provides interventions and support for households with high risk and vulnerability based on identified needs. 

*  Manages incoming and outgoing referrals within the Stable Housing Network for prevention services such as targeted homelessness prevention support and emergency rental assistance. 

*  Collaborates with Stable Housing Network partners, including Housing Resource Specialists and CSB's Homelessness Prevention Network Manager. 

*  Connects clients with appropriate resources such as food pantries, childcare, legal aid, and flexible funds to assist housing stabilization. Provides housing resource information and navigational support for clients and other staff. 

*  Maintains and updates housing-related information and resources. Performs data entry in accordance with Homeless Management Information System (HMIS) data standards. 

*  Participates in professional development activities, attending relevant trainings and maintaining credentials, including HMIS End User License and Housing Problem Solving certification.

Qualifications:

Education Requirement: 
Bachelor's Degree in social work, counseling, or related field, required. 

Licensure Requirement: 
Valid Ohio driver's license and proof of auto insurance as required by hospital policy. Must pass motor vehicle background inspection, insurance eligibility, driving qualifications, and training set forth by Nationwide Children's Hospital and maintain qualification of insurance guidelines. 

Certifications: 
Housing Problem Solving certification or completion of training after hire, required. 

Skills: 

*  Working knowledge of effective social service interventions, particularly crisis-oriented interventions, and approaches to program and system collaboration. 

*  Knowledge of issues related to homelessness and homelessness prevention, including causes of homelessness and characteristics of persons who experience homelessness. 

*  Knowledge of issues and interventions relevant to families and individuals most at-risk of literal homelessness. 

*  Working knowledge of leases, landlord-tenant rights, and eviction proceedings to make appropriate referrals for legal assistance and mediation services. 

Experience: 

*  Two years of experience in social services, or related field, preferred. 

*  Experience working in a not-for-profit setting, preferred. 

*  Experience working with people experiencing homelessness or housing instability or with other disadvantaged populations, preferred. 

*  The significance and expertise of a candidate that has experienced homelessness or housing instability will be considered.
