Qualifications:
The Director of Memory Care is responsible for directing the operations of our Memory Care neighborhood (Legacy), a memory care program, working closely with residents, staff, and stakeholders to create a vibrant and inclusive living environment that meets the needs and preferences of people living with dementia.
Key Responsibilities
* Direct and oversee the general daily operations of the Legacy neighborhood utilizing industry and company dementia best practices.
* Educate family members with the Family Orientation Guide, schedule and lead resident first meetings/service plans and assist family members through the campus continuum as outlined in the company's Legacy move in/move out criteria.
* Maintain a dementia-safe environment through daily resident rooms, common areas, and security rounds.
* As the "Dementia Champion" and subject matter expert, provide education to family members and the community in dementia-related topics.
* Oversee, communicate, and ensure Level of Care completion and follow-up per company policy.
* Complete Social Services and Life Enrichment assessments and documentation for Legacy residents.
* Market and promote the Legacy Neighborhood to potential referral sources and develop relations with local dementia advocacy groups to achieve occupancy goals.
* Represent the Legacy Neighborhood at meetings to provide support for planning and conducting various functions within the neighborhood such as activities, outings, event planning, meal /food requests, etc.
Qualifications
* Bachelor's degree from an accredited educational institution in a health care, mental health or social service profession or be a licensed Health Facility Administrator (HFA)
* Minimum of one (1) year work experience with dementia or Alzheimer's residents, or both within the past five (5) years
* Three (3) years' dementia care experience in a healthcare, senior living industry, or long-term care environment preferred
* Certified Dementia Practitioner credential (CDP) preferred
