Responsibilities:

The Director, Clinical Services (DCS), is responsible for the planning, direction, coordination, supervision, evaluation and provision of all nursing services provided by the assigned facility location. The DCS oversees the coordination, continuity, and quality of clinical care, as well as the management of patient services and monitors, evaluates, and takes action on clinical measures of performance. The DCS participates in the recruitment of associates, and provides the supervision necessary to facilitate the provision of quality care and patient satisfaction; and leads the education, training, and development of associates.

Leadership & Operations
* Direct and manage clinical operations to ensure the delivery of safe, effective, and compassionate care.

* Act as a clinical liaison for interdisciplinary teams, patients, and families.

Staff Management & Development
* Recruit, train, and retain top clinical talent.

* Set clear expectations and performance goals; deliver regular coaching and feedback.

* Cultivate a positive, engaged, and high-performing work environment.

* Lead educational initiatives including orientation, in-service training, and case management support.

Clinical Oversight & Quality Assurance
* Ensure compliance with all state, federal, and accreditation standards (ACHC).

* Monitor and improve clinical outcomes using data tools like SHP and HCHB.

* Oversee accurate documentation, OASIS scoring, and physician order management.

* Implement and lead QAPI programs to drive ongoing quality improvement.

Patient-Centered Care
* Ensure individualized care planning that aligns with patient goals and best practices.

* Conduct supervisory visits and support clinical fieldwork when necessary.

* Maintain continuity of care through effective coordination and communication.

Administrative & Regulatory
* Participate in budget planning, financial reviews, and strategic initiatives.

* Support regulatory readiness and may serve as Administrator of Record.

* Provide operational backup to the Director of Operations as needed.

Qualifications:

* Current RN, PT, or OT license in good standing.

* Minimum 3 years of clinical leadership experience.

* At least 2 years of home health or related clinical experience.

* Strong knowledge of regulatory compliance, OASIS, and outcome-based care models.

* Excellent interpersonal, organizational, and leadership skills.
