Qualifications: The Master Social Worker (MSW) is responsible for providing psychosocial support to patients undergoing dialysis treatment and counseling families on lifestyle adjustments and coping strategies. Key Responsibilities * Assess patients' financial, medical, social, and familial needs. * Complete psychosocial assessments and recommend changes in treatment based on the patient's current needs. * Provide case work and group work services to patients and their families in dealing with the special problems associated with end stage renal disease. * Participate in team review of patient progress and completing monthly care plans on patients. * Attend monthly Quality Assurance Performance Improvement (QAPI) meetings and participate in the final development of patients' Plan of Care on new patients. * Provide pre-transplant counseling for patients and family members and make necessary arrangements with the transplant center. * Provide insurance information on new patients and transients to the business office. * Participate in periodic home visits to complete initial assessments. Qualifications * Master's degree in social work (MSW) with a specialization in clinical practice * At least two (2) years of experience as a social worker, with one (1) year in a dialysis unit or transplantation program preferred * Must meet the licensure requirements, as required, in the state of practice
