Qualifications: The Dialysis Dietician provides comprehensive nutritional care and assessment for dialysis patients in accordance with standard regulations. Conducting thorough nutrition assessments, reviewing patient weight and lab results, and making dietary and medication recommendations to the Dialysis Nurse-RN and Dialysis Manager. Key Responsibilities * Completes comprehensive Nutrition/Dietitian assessment per standard regulations within the dialysis EHR system for all dialysis patients. * Completes monthly report cards, and conducts patient education as required by the report card. * Reviews patient labs and provides recommendations to the Dialysis Nurse-RN and DHS of Dialysis. * Participates in in-person care plan meetings and identifies resources for patients' meal planning. * Works with health center dietitian for patient interventions. * Reviews with the nephrology labs that have to be re-drawn mid-month and makes appropriate recommendations. Qualifications * Bachelor's Degree * Active Registered Dietician License in the state of practice * 0-1 years of relevant experience
