Responsible for identifying appropriate candidates for home care services. Evaluates patients referred for treatment to determine appropriateness for admission. Position will provide exceptional customer service while participating in the discharge planning process and evaluating patients for agency services. Ensure referral source and patient needs are met. Completes agency referrals, providing accurate demographic information and appropriate information for care. Position will provide exceptional customer service while participating in the coordination of care/scheduling process for admissions to homecare. Problem solving through excellent written and verbal skill. Data entry and clerical duties.

Schedule: Monday-Friday, 8:00 AM-5:00 PM

Qualifications:

In addition to bringing humankindness to the workplace each day, qualified candidates will need the following:

* High school diploma/GED, required
* Associate degree in a health care related field, preferred
* 1 year of home care, hospice, home oxygen, HME, or home infusion experience preferred.
* Prior customer service/relations and or telephone sales experience preferred.
* Excellent customer service skills and a commitment to meet the needs of all customers and patients.
* Knowledge of comprehensive home care services and supporting community organizations, strong organizational, and analytical, and communication skills preferred.
